In this, you have to give your name and gas consumer number, and you have to give the name of your husband or father.
Apart from this you also have to provide documents as address proof.
For proof of address, apart from documents like an Aadhaar card, driving license, lease agreement, or voter ID card, you have to provide a photocopy of one of the documents like a passport and ration card.
